Myth: Creatine Causes Bloating and Water Retention

The fact: Creatine does pull a small amount of extra water into muscle cells early on, which is why some people notice the scale tick up slightly in the first week or two. But that 2021 review of the evidence found this is intracellular water, inside the muscle, not the puffy, under-skin water retention people associate with "bloating." Over longer periods, several studies found no meaningful change in total body water relative to muscle mass. Myth: Creatine Is a Steroid

The fact: This one persists because both are associated with muscle and performance, but the comparison doesn't hold up chemically. Anabolic steroids are synthetic versions of testosterone with a specific steroid ring structure. Creatine has an entirely different molecular structure and isn't a hormone at all. It's a compound your own body already makes in the liver, kidneys, and pancreas from amino acids, and one you're already consuming if you eat meat or fish. Myth: Creatine Causes Hair Loss

The fact: This myth traces back almost entirely to one small 2009 study that measured DHT (a hormone linked to hair loss) in rugby players on a high-dose creatine protocol, and found a statistical increase. It didn't measure hair loss or hair follicle health directly, and the creatine group happened to start with unusually low baseline DHT, which exaggerated the apparent change. The 2021 expert review concluded the current evidence does not show creatine increases testosterone, free testosterone, or DHT. More recent, purpose-built trials have gone further: a 12-week randomised controlled study measuring hair density and shedding directly found no significant effect at all. Decades of research, one outlier study, and it still won't die online.

Myth: Creatine Causes Dehydration and Muscle Cramps

The fact: This is another myth that sounds logical (creatine moves water into muscle, so surely it must dehydrate you elsewhere) but doesn't hold up experimentally. Clinical research reviewed by Candow and colleagues found no evidence that creatine supplementation causes dehydration or cramping at standard doses. Normal hydration habits for anyone training regularly are all that's needed.

Myth: Creatine Makes Women Bulky

The fact: Dr Stacy Sims addresses this directly with active women: muscle size is driven by training stimulus, nutrition, and hormones, not by creatine itself. What creatine does support is strength and power output during training, which is a very different thing from "bulk." Sims also points out that women naturally hold around 70 to 80% less creatine in muscle than men, which is part of why she considers supplementation particularly valuable, rather than something to be cautious of, for active women. The 2021 misconceptions review reached the same conclusion from the wider research: creatine has therapeutic potential across the female lifespan with little to no meaningful side effects.

Myth: Creatine Only Works for Bodybuilders and Young Athletes

The fact: Much of Dr Darren Candow's own research focuses specifically on older adults, not gym bros. His work shows creatine, combined with resistance training, provides real anti-sarcopenic benefits for older adults, meaning it helps counter the natural, age-related loss of muscle mass and strength. Interestingly, one nuance from this research didn't make it into most gym folklore: a 2021 meta-analysis co-authored by Candow's colleague Dr Scott Forbes found that a loading phase may actually matter more for older adults specifically, particularly for building lower-body strength, even though most younger adults don't need one at all. Bone density benefits are more modest ("more hype than hope" according to Candow's own framing), but the muscle and functional benefits for older adults are well supported.

Myth: Creatine Damages Your Kidneys

The fact: This is the myth that refuses to go away, largely because creatine breaks down into creatinine, the exact marker doctors use to check kidney function on a blood panel. Seeing that number rise looks alarming if you don't know why it's happening. But the 2021 review, and more than three decades of research and safety monitoring behind it, found no evidence that creatine causes kidney damage in healthy people at recommended doses. It's also worth knowing that serum creatinine isn't actually the most reliable way to assess kidney health on its own, cystatin C and glomerular filtration rate (GFR) are considered more accurate markers, and a rise in creatinine from supplementation is simply expected metabolic turnover, not organ damage.

Myth: All Creatine Is Basically the Same

The fact: This is where the myth-busting gets practical. The 2021 review confirmed that while some newer forms of creatine claim better solubility, creatine monohydrate remains the gold standard, the most researched and most effective form available, and nothing has been shown to outperform it.
